Before You Begin

Your proxy helps the meeting proceed.

If you cannot attend, a properly completed proxy may help establish quorum and, depending on your selection, authorize an eligible representative to act on your behalf. Follow the official meeting notice and your association’s governing documents.

Quorum Notice
There must be a quorum as stated by your association bylaws in order to conduct the Annual Meeting. If you are unable to attend this Meeting, please complete the Proxy below so that a quorum will be represented at the Meeting. NOTE: YOU MAY EITHER GRANT A GENERAL PROXY OR A PROXY SOLELY FOR THE PURPOSE OF BEING COUNTED FOR A QUORUM. It is most important that you either attend the Meeting or fill out this Proxy so that the Association can function.

Hereby grant this Proxy in accordance with Article III, Section 3.06 and 3.08 of the Bylaws of the Association Named Above, and with the Policy resolution of the Board of Directors of Association Named Above, Regarding Procedures for Submitting a Written proxy to be presented at a Meeting. This Proxy carries with it the intent of the undersigned to be represented at the meeting of members specified herein by the individual to whom this Proxy is issued and specified.
